We collect the categories below for the stated business purposes. We retain each category only as reasonably necessary for those purposes, generally no longer than 24 months after the most recent interaction unless law, security, dispute resolution, or an opt-out record requires a longer period.
- Identifiers: name, email address, telephone number, ZIP code, Internet Protocol address, and similar online identifiers.
- California customer-record information: name and telephone number.
- Internet or electronic activity: browser type, pages requested, referring page, access time, and interactions with the website.
- Consent and communication records: form submission time, consent version, communication preferences, and opt-out records.
- Approximate geolocation: a general location that may be inferred from an IP address or ZIP code; we do not request precise geolocation.
We use these categories to respond to requests, communicate with your consent, operate and secure the site, prevent abuse, maintain compliance records, and satisfy legal obligations. We do not request medical records, Social Security numbers, account credentials, or government identification through the website.
2. Sources of personal information
We collect information directly from you when you submit a form or communicate with us, and automatically from your browser and our hosting systems when you use the site.
3. Disclosures for a business purpose
During the preceding 12 months, we may have disclosed the categories listed above to service providers that supply hosting, security, technical support, and communications functions. They are permitted to process information only to provide contracted services to us. We may also disclose information when required by law or to protect rights and safety.
4. No sale or behavioral-advertising sharing
SSDI Pathway does not sell personal information for money or other valuable consideration and does not share personal information for cross-context behavioral advertising, as those terms are defined by the CCPA. We have not knowingly sold or shared personal information of consumers under 16.
5. Sensitive personal information
Our website form is designed not to collect sensitive personal information as defined by the CCPA. Do not submit medical records, Social Security numbers, financial account credentials, government identifiers, or precise geolocation through this website. Because we do not use or disclose sensitive personal information for purposes that trigger a right to limit under the CCPA, we do not provide a “Limit the Use of My Sensitive Personal Information” link.
6. California privacy rights
Subject to verification and legal exceptions, California residents may have the right to:
- know the categories and specific pieces of personal information collected about them;
- know the sources, business purposes, and categories of recipients;
- request deletion of personal information;
- request correction of inaccurate personal information;
- opt out of sale or sharing (we do not sell or share as described above);
- limit certain uses of sensitive personal information when applicable; and
- receive equal service and not be retaliated against for exercising a privacy right.
7. How to submit a request
Email privacy@ssdipathway.com with the subject “California Privacy Request.” State whether you are requesting access, correction, or deletion and provide the email address and telephone number used with SSDI Pathway. Do not send identification documents, medical records, or Social Security numbers with your initial request.
We will confirm receipt and may ask for information reasonably necessary to match you with our records. We use verification information only to evaluate and fulfill the request. If we cannot verify a request, we will explain the limitation. Where applicable, you may appeal a decision by replying to our response with “Appeal” in the subject line.
8. Authorized agents
An authorized agent may submit a request for you. We may require proof of the agent’s signed authority and may ask you to verify your identity directly, unless the agent holds a qualifying power of attorney. Agents should use the email process above and identify the request as an “Authorized Agent Request.”
9. Financial incentives
We do not offer financial incentives or price differences in exchange for personal information.
